The evaluation of adrenal insufficiency in suspected adrenal biosynthetic defect or adrenocorticotropic hormone (ACTH) unresponsiveness.


Test includes three serum cortisol determinations--one at 0 minutes, one at 30 minutes, and one at 60 minutes.

Specimen Collection and Handling

1. Draw baseline blood in a serum gel tube.

2. Immediately after drawing baseline, clinician administers 0.25 mg ACTH (Cortrosyn) injection.

3. Draw second blood specimen 30 minutes post injection.

4. Draw third blood specimen 60 minutes post injection.

5. Label tubes with collection time.

To Process:

1. Send 1.0 mL of serum from each specimen (pre and post injection).

2. Label tubes with the collection time.

Clinical Significance

No sample should be collected on patients receiving therapy with high biotin doses (i.e. biotin therapy for Multiple Sclerosis or oncology patients; skin, hair and nail supplements, or multivitamins containing > 5 mg/day) until at least 12 hours after the last biotin administration. If unsure, or if the clinical picture does not fit the results please contact the laboratory . We have methods to check for biotin interference as well as for any other interferences and for the accuracy of the results.

Biotin interference would falsely increase the result of this assay.
