Therapeutic ranges: Trough: 5-15 ug/ml.
For patients on meningitis, osteomyelitis, endocarditis, and pneumonia therapy: Vancomycin Trough: 15-20 ug/mL

Concentrations greater than 50.0 ug/mL are considered potentially toxic.
