Test Code TXPBN TOXICOLOGY PROFILE WITH REFLEX TO CONFIRMATION AND A BUPRENORPHINE NEGATIVE REFLEX , URINE

Important Note

This panel does not detect synthetic opiates.  Please order separate tests/panels that contain the specified drug name in the title:

  • Meperidine (Demerol)
  • Tapentadol (Nucynta)
  • Tramadol (Tradol, Ultram, Ultracet)

Performing Laboratory

NorDx Laboratories

Useful For

Screening of the following drugs/drug classes: amphetamines, barbiturates, benzodiazepines, cocaine metabolite, methadone, methadone metabolite, fentanyl, MDMA (Ecstasy), opiates, oxycodone, phencyclidine (PCP), propoxyphene, tetrahydrocannabinol (THC)

This panel also screens for the presence of buprenorphine (an opiate) in urine by EIA at a cutoff concentration of 5 ng/mL.

Positive test results are confirmed and/or quantitated at an additional charge.

A “not detected” buprenorphine result will be confirmed and quantitated if detectable by LC-MS/MS at an additional charge.

Method Name

Cloned Enzyme Donor Immunoassay (CEDIA)

Enzyme Immunoassay (EIA)

Gas Chromatography-Mass Spectrometry (GC-MS) (If indicated)

Liquid Chromatography-Tandem Mass Spectrometry (LC-MC/MS) (If indicated)

Reference Values

Toxicology Profile: Not Detected

Screening cutoff concentration:

Amphetamines: 500 ng/mL

Barbiturates: 200 ng/mL

Benzodiazepines: 200 ng/mL

Buprenorphine: 5 ng/mL

Cocaine (benzoylecgonine – cocaine metabolite): 300 ng/mL

Fentanyl: 1 ng/mL

Methadone: 300 ng/mL

Methadone metabolite: 1000 ng/mL

MDMA: 500 ng/mL

Opiates: 300 ng/mL

Oxycodone: 100 ng/mL

Phencyclidine: 25 ng/mL

Propoxyphene: 300 ng/mL

Tetrahydrocannabinol carboxylic acid: 50 ng/mL

The presence of buprenorphine at 5 ng/mL or norbuprenorphine at 5 ng/mL is a strong indicator that the patient has used buprenorphine.

For help with interpretation please call: 877-323-0045

Please see test code BUPMT for additional buprenorphine confirmatory testing information.

Advance Beneficiary Notice Requirements

This test, when ordered on Medicare patients, is subject to the National Coverage Determination (NCD) policies. Please verify that the diagnosis code (ICD code) you have chosen demonstrates medical necessity for the test as documented in the physician’s patient record. The CMS web site is available to assist you with this verification. A properly executed Advance Beneficiary Notice (ABN) must be submitted with the specimen if medical necessity is not demonstrated by the ICD code chosen.
