Therapeutic concentration: 15.0-40.0 ug/mL
Toxic concentration: >60.0 ug/mL
A correlation between serum phenobarbital concentrations and therapeutic response has not been clearly demonstrated. To treat or prevent seizures in epilepsy, a trough serum concentration between 15-40 ug/mL is recommended. For other indications, including alcohol withdrawal or status epilepticus, higher trough serum concentrations may be necessary. Phenobarbital as a half-life of 80 hours so steady-state is achieved after at least two weeks of therapy.

Clinical Significance
Phenobarbital is a general central nervous system suppressant that has proven effectiveness in the control of generalized and partial seizures. It is frequently co-administered with phenytoin for control of complex seizure disorders and with valproic acid for complex parietal seizures.
