1. Aseptically collect specimen from affected area (external or middle ear).
2. Submit specimen on swab in culture transport medium.
To Process:
1. Maintain sterility and forward promptly.
2. Send at ambient temperature.
3. Label specimen with patient’s name (first and last), date of birth or medical record number, date and time of collection, collector’s initials and SPECIMEN SOURCE
SPECIMEN SOURCE IS REQUIRED ON REQUEST FORM FOR PROCESSING.
