Specimen Collection and Handling
1. Swab vaginal introitus and rectum with a single swab (do not use a speculum) and place swab in a culture transport media tube.
2. Send specimen at ambient temperature.
3. Maintain sterility and forward promptly.
4. If detection of any other genital pathogens is desired, use a speculum and appropriate transport media to collect additional specimen(s).
To Process:
1. Label tube with patient’s name (first and last), date of birth or medical record number, date and time of collection, collector’s initials and specimen source.
2. Specimen source is required on request form for processing.

Clinical Significance
Indications:
Group B Streptococci are universally susceptible to penicillin, ampicillin and cefazolin. Guidelines published jointly by the Centers for Disease Control and Prevention, American College of Obstetricians and Gynecologists and the American Academy of Pediatrics recommend treatment of most patients during labor with one of these antibiotics.
Antibiotic susceptibility testing is only recommended for patients who are allergic to penicillin and who are at high risk of anaphylaxis if treated with one of these antibiotics.
The guidelines recommend intrapartum treatment of patients at high risk of anaphylaxis with clindamycin or erythromycin if the organism tests susceptible to both antibiotics. If the organism is resistant to either clindamycin or erythromycin, treatment with vancomycin will be necessary.
Positive culture may reflex to antibiotic susceptibility testing at an additional charge.
